Objective To evaluate the efficacy of emergency surgery for acute cervical spinal cord injury. Methods A data of 42 patients with cervical spinal cord injury treated in our depart ment from Jan 2000 to Jan 2011 were retrospectively studied. Of 42 cases, 18 underwent emergency surgery (time from injury to surgical ≤24h), and 24 cases received non-emergency surgery (>24h). Neurological functions, postoperative complications and hospital stay were analyzed statistically. Results The sensory and motor function got more significant improvement in emergency surgery group than those in controls. The incidence rate of complications, ICU and hospital stay were of emergency surgery lower than of non-emergency surgery group. Conclusion Emergency surgical decompression for cervical spine fracture with acute cervical spinal cord injury is feasible in the recovery of neurologic outcome, and reduces perioperative complications.

Objective To investigate the imaging features of gallbladder carcinoma in magnetic resonance imaging(MRI), and to improve the diagnosis of the gallbladder carcinoma. Methods 47 patients with clinical diagnosis of gallbladder carcinoma were studied with MRI. 43 cases were verified by pathology. The MRI signs of the gallbladder carcinoma cases were analyzed and compared with the surgical and pathological findings. Result 12 cases were wall-thickening type; 19 cases were intraluminal type; 16 cases were mass type.The lesions were slightly hypointense or isointense signal in T1WI, and slightly hyperintense or hyperintense signal in T2WI. The model of enhancement isprogressive or continued.There were 20 cases combined with dilated intra/extrahepatic bile duct. The level of the obstructions were common hepatic duct in 80% cases. There were25 cases with directly invading adjacent liver parenchyma, 6 involving duodenum, 4 involving omentum and 3 involving pancreas. There were 24 cases with lymph node metastasis, including 9 cases in N1 stage and 15cases in N2 stage. There were 10 caseswith liver metastasis, all of which were diagnosed correctly.The accuracy of clinical stages according to MRI findings was 90.7% compared with surgical stages. Conclusion MRI could clearly show the gallbladder carcinoma, and accurately evaluate the spread of invasion and metastasis. MRI is helpful to making exact clinical tumor stages, which is essential to making therapeutic option.

Objective To study how to select the most appropriate treatment in treating choledocholithiasis with cholecystolithiasis and its curative effect. Methods Clinical data of 52 patients with choledocholithiasis and cholecystolithiasis in our hospital from October 2010 to October 2011 were retrospectively analyzed .Through the different treatment methods we analyzed the effect and safety . Results In this group of 52 patients ,24 patients were treated by LC+LCBDE, 2 of them converted to laparotomy. 25 patients were treated by EST+LC; 3 patients were treated by ENBD+LC+LCBDE. None of the 52 patients appeared the complication of bile leakage, bleeding, pancreatitis, cholangitis or residual stones . Conclusion Selecting the optimal combination of the three endoscopes after examinations to deal with the disease has concentrated the superior of the three endoscopes . It's the least traumatic way to treat the disease , the effect is positive.
