【Abstract】 Objectives To investigate the effects of pneumoperitoneum and infusion with hydroxyethyl starch (HES) l30/0.4 solution intraoperation on hemorheological parameters of patients underwent laparoscopic cholecystectomy (LC). Methods Sixty patients underwent LC were taken blood sample for determination of hemorheology variables on the morning of the operation day , after pneumoperitoneum, after infusion with HES 130/0.4 solution (or the Ringer’s lactated solution) and the first morning after operation. The hemorheological parameters includs: WBV, PV, HCT, ESR, ESR K, RI, EAI. Results WBV, PV, RI, EAI increased significantly after pneumoperitoneum. After using HES 130/0.4 solution, WBV, HCT, EAI, ESR K of the patients decreased significantly, the treatment groups had more decreasing amplitude than the control groups. The non-elderly patients recovered on the first morning after operation, but WBV, PV, HCT, EAI of elderly patients are still higher. Conclusions The pneumoperitoneum can affect the hemorheological characters of patients(especially the elderly patients) underwent LC. Loading infusion HES 130/0.4 solution can lower the blood viscosity, ameliorate the microcirculation and decrease the occurrence of thrombosis complications.

【Abstract】 Objective To construct the restructuring lentiviral expression vector that carries miR-122, and lay a foundation for us to study the functions and mechanisms of miR-122. Method The pre-miR-122 gene was amplified from mouse genomic DNA by PCR, which was cloned to the retroviral vectors of pCDH-CMV-MCS-EF1-copGFP and sequenced. After viral packaging, the NIH3T3 cells were transfected with recombinant lentiviral vector. The titer of virus was detected by green fluorescent protein determination. Finally the expression of miR-122 of hepatic progenitor cells (HPCs) infected with recombinant lentiviral vector was analyzed by using real-time quantitative PCR. Results PCR and DNA sequencing assays demonstrated that the recombinant lentiviral vector of pCDH-CMV-miR-122-EF1-copGFP was successfully constructed, and the titer of concentrated virus was (1~ 5)×106 IU/mL. The level of miR-122 expression in HPCs was significantly increased after infection with pCDH-CMV-miR-122-EF1-copGFP. Conclusion Mouse miR-122 recombinant lentiviral vector: pCDH-CMV-miR-122-EF1-copGFP was successfully constructed, which resulted in high miR-122 expression in HPCs. Our results lay a good foundation to explore the functions and mechanisms of miR-122.

【Abstract】 Objective To assess the feasibility of endoscopic endoluminal surgery. Methods 4 pigs were used in the experiments.Percutaneous channels entering the gastric cavities were established. Results All the 4 pigs had tolerated the surgery smoothly.Postoperative examinations by laparotomy found no leakageon suture or injuries of organs.The time for establishing endo-gastric channels surgery were 15-30min. Conclusions Endoscopic endoluminal surgery is technically feasible.

Aim To explore the feasibility of a simultaneous entirely laparoscopic procedure, in the light of the benefits of laparoscopy in both rectal and liver surgery, and discussed the surgical techniques and benefits of this strategy. Methods One case is presented of totally laparoscopic left lobe liver resections associated with laparoscopic colorectal resections for synchronous liver metastases with the emphasis on the technical aspects. Duration of surgery, blood loss and post-operative outcome were evaluated. Results Laparoscopic left hepatectomy with simultaneous colon resection for liver metastasis was feasible and safe. The mean duration of surgery was 190 min with a mean estimated blood loss of 300 ml. The postoperative course was uneventful. Discussion In selected patients, laparoscopic major hepatectomies for unilobular synchronous metastases can be safely performed simultaneously with colorectal surgery.

[Abstract] Objective To summarize abdominal multiple tubed pattern flaps used for treatment of whole-finger degloving injury of multiple fingers(2 to 3). Methods Since 2006, 11 patients with whole-finger degloving injury of multiple fingers were treated with transfer of random abdominal tubed pattern flaps, abdominal wound were sutured at the first-stage operation and fingers were dissected with pedicels after 5 to 6 weeks. Results All the skin flaps survived. Two fingers of 2 cases had distal cutaneous necrosis after dissecting with pedicels and were scar healing after dressing change finally. Follow-up from 5 to 36 months showed the active motion of metacarpophalangeal articulations averaged 60°?, and interphalangeal joints averaged 15°. All fingers could perform grasping , therefore patients could handle with daily life and were satisfied with the contour of fingers. Conclusion Treatment of whole- finger degloving in jury of multiple fingers(2 to 3) with abdominal multiple tubed pattern flaps can restore the shape and function of hand to the greatest extent and is a practical safe and convenient operative method. 【Abstract】Objective: To investigate the value about the treatment of traumatic fracture of ribs using wrap-around plate internal fixation. Methods: From March 2009 to December 2011 ,the operative outcomes of 42 patients with traumatic fracture of ribs without opening the thoraxes but using wrap-around plate internal fixation were analyzed retrospectively. Results: The operation is simple, the operation time is about 20 ~ 80 minutes.The average hospital stays are about 10 days. The result is good. It can lead to anatomical reduction and remedy deformity, compared with the preoperative chest pain in patients ,they get apparent ease. Fewer trauma, fewer complications, Malposition or fracture again are not found any more. Conclusion: The treatment of traumatic fracture of ribs using wrap-around plate internal fixation is a relatively safe and effective approach with fewer Trauma.

【Abstract】 Objective To observe the effects of dexmedetomidine on propofol consumption, postoperative pain and agitation in patients undergoing tympanoplasty. Methods Sixty ASA I or II patients scheduled for tympanoplasty were randomly allocated into two groups (n=30 each). Dex was administrated 1μg.kg-1 intravenously 10mins before induction and then continuously infused with a rate of 0.3μg.kg-1.h-1 in group D while normal saline was adopted instead of Dex in group C. In both groups, anesthesia was induced and maintained with propofol and remifentanil; target concentration of propofol was regulated to keep Narcotrend index (NI) within (46-20) range; BP, HR, propofol consumption, RS, RSS, VAS and fentanyl requirements were recorded. Results Propofol consumption during surgery and fentanyl requirements, RS, VAS score during anesthesia recovery were less in group D than group C while RSS score was higher in group D than group C(P<0.05). Conclusion Dexmedetomidine can effectively reduce propofol consumption and alleviate the incidence and degree of postoperative pain and agitation in patients undergoing tympanoplasty.
