Objective: Analyse clinical pathological characteristics in different tumor of periampullary and its correlation with clinical prognosis. Methods: Retrospective review of clinicopathological and follow-up data in 110 patients with pancreaticoduodenectomy sufferring from periampulary cancer. Results:The operative time ,blood loss ,positive rate in surgical margin and lymph node metastsis is significant high in pancreatic caner group than non-pancreatic cancer group.Tumor stage,positive surgical margine and lymph node metastasis is significant independent risk factors for patients with pancreaticoduodenectomy.There was significant difference in pancreatic carcinoma and non-pancreatic carcinoma with pancreaticoduodenectomy. Conclusion:Tumor stage,positive surgical margine and lymph node metastasis is significant independent risk factors for patients with pancreaticoduodenectomy.Prognosis of non-pancreatic carcinoma was better than pancreatic carcinoma with pancreaticoduodenectomy

[Abstrcat]: Objective: To investigate the effects of different-dose ulinastatin(UTI) on tumor necrosis factor-alpha (TNF-a) and interleukin-6(IL-6) of old surgical abdomen patients in surrounding operation time. Method: 120 old surgical abdomen patients were randomly divided into 4 groups(n=30):the blank control group, UTI 5000U/kg,10,000U/kg and 20,000U/kg. The blood from central vein was gained at various time points. TNF-a and IL-6 in every blood sample were determined.Observe the therapeutic in each group. Result: TNF-a and IL-6 in every treatment group were obviously lower than which in blank control group (p<0.05); TNF-a and IL-6 in every treatment group at the same time points decreased progressively compared with U1(UTI 5000U/kg) (p<0.05). The incidence rate of multiple organ dysfunction syndrome(MODS) was no significant difference between U1(UTI 5000U/kg) and blank control group(P > 0. 05),and which in U2( UTI 10,000U/kg)and U3(20,000U/kg ) were obviously lower than which in blank control group (p<0.05);The case fatality rate in each treat group was no significant difference compared with which in blank group(P > 0. 05). Conclusion: UTI shows dose-dependent effect inhibiting the delivery of cytokine such as TNF-a and IL-6,and obviously lessen the systemic inflammatory reaction syndrome(SIRS),and obstruct multiple organ dysfunction syndrome(MODS),which conduces to help the sufferer to pass the surrounding operation time.

Objective To explore the expression and its significance of CD4+CD25+ regulatory T cells (Tr) in pathogenesis of chronic abacterial prostatitis patients (CAP). Methods 20 CAP patients and 10 volunteers were taken into this study and their peripheral blood was collected for testing the Tr through flow cytometry. Foxp3, the important regulatory factor of Tr, was detected by RT-PCR. The Tr’ cytokines including IL-10 and TGF-β1 in both peripheral blood (PB) and prostatic fluid(PF) were measured by ELISA. Routine blood tests (BT), C-reaction protein (CRP) and erythrocyte sedimentation rate (ESR) were also compared as the inflammatory indexes. Results Compared with the volunteers, percentage of Tr in the CAP patients was significantly lower so was the Foxp3 (P=0.013 and 0.000 respectively). About the cytokines, the IL-10 and TGF-β1 in both PB and PF of the CAP group were significantly lower than that in the volunteers, so was the monocyte in PB (MONO). But there was no statistically difference of WBC, NEU, LYM, CRP and ESR between the two groups. Pearson correlation analysis showed that MONO correlated with the quantities of Tr, Foxp3, IL-10 and TGF-β1. Conclusion Decreased Tr resulting insufficient of immunodepression and degressive IL-10 causing proliferation of MONO, which leaded to the excessive antigen presenting and immune injury, maybe a key role in pathogenesis of chronic abacterial prostatitis.

Objective: To investigate the principle of diagnosis and treatment of testicular mature teratoma and improve the understanding of the disease. Methods : Clinical data of 2 cases of testicular mature teratoma in adult were retrospectively analyzed, including clinical symptom, examinations and the treatment, and this disease was reviewed based on literatures. Results: The 2 patients were all childhood onset without any special therapy for no obvious symptoms. They were performed simple tumor enucleation with testis retained because of the increasing of teratoma recently. There was no recurrence after operation. Conclusion: This disease is the lack of special symptoms, Color Doppler ultrasonography of scrotal and CT examinations are helpful to the diagnosis, testicular biopsy can be diagnosed. We can use simply tumor enucleation, and decide whether to undergo radical resection of the testes according to intraoperative pathology results.
