Abstract: 【Abstract】 Objective To observe the effect of respiratory function training on postoperative respiratory function and sputum excretion in lung cancer patients with pulmonary lobectomy. Methods Eighty lung cancer patients received pulmonary lobectomy in our hospital from May 2015 to November 2016 were selected as research object,in which 40 cases received routine nursing from May 2015 to February 2016(control group),and received health education,oxygen uptake,aerosol inhalation before and after surgery. Another 40 cases were assigned to observation group and accepted preoperative inspiratory muscle training on the basis of routine nursing from March 2016 to November 2016. The respiratory function pre?and post?operation for 7 d (maximal voluntary ventilation(MVV),forced vital capacity(FVC),forced expiratory volume in one second(FEV1),PaO2,PaCO2),sputum excretion amount,postoperative hospital stays,incidence of complications of two groups were recorded and compared between two groups. Results After surgery,the MVV%,FVC%,FEV1%,PaO2 levels of two groups were lower than before surgery,and PaCO2 level was higher than before surgery(P < 0.05);After surgery,the MVV%、FVC%、FEV1%、PaO2 levels of control group were lower than those of research group(P < 0.05). After surgery,there was no statistical difference in the PaO2 level compared between two groups (P > 0.05). After surgery, the sputum excretion amount of control group was lower than that of research group, and postoperative hospital stays were higher than that of control group(P < 0.05);After surgery,the total incidence of complications of control group was higher than that of research group(P < 0.05). Conclusion Preoperative inspiratory muscle training can decrease the postoperative complications for lung cancer patients, which can improve respiratory function, promotion sputum excretion, shorten postoperative hospital stays,reduce risks of complications,and promote patients′ recovery.

摘要: 【摘要】目的 研究呼吸功能训练对肺癌肺叶切除术患者术后呼吸功能及排痰的影响。方法 选取我院2015年5月~2016年11月收治的80例接受肺叶切除术治疗的肺癌患者作为研究对象。将2015年5月至2016年2月期间接受常规护理的40例患者纳入对照组,手术前后接受健康宣教、吸氧、雾化吸入等常规护理;将2016年3月至2016年11月期间在常规护理基础上接受术前呼吸功能训练的40例患者纳入研究组。比较两组患者术前、术后7d的呼吸功能包括每分钟最大通气量(MVV)、用力肺活量(FVC)、1秒用力呼气容积(FEV1)、动脉血氧分压(PaO2)、动脉二氧化碳分压(PaCO2)、排痰量、术后住院时间、并发症发生率。结果 术后两组患者MVV%、FVC%、FEV1%、PaO2水平均低术前,PaCO2水平均高于术前,差异有统计学意义(P<0.05);术后对照组MVV%、FVC%、FEV1%、PaO2水平低于研究组,差异有统计学意义(P<0.05);两组患者术后PaCO2水平比较,差异无统计学意义(P>0.05);对照组患者术后排痰量低于研究组,术后住院时间高于研究组,差异有统计学意义(P<0.05);术后对照组患者并发症总发生率高于研究组,差异有统计学意义(P<0.05)。结论 呼吸功能训练能降低肺叶切除术对肺癌患者术后呼吸功能的影响,改善呼吸功能,促进排痰,缩短术后住院时间,降低并发症发生风险,促进患者康复。
