多节段骨质疏松性椎体压缩骨折一期强化治疗的疗效及安全性评估

摘要: 【摘要】〓目的〓探讨骨质疏松性椎体压缩骨折采用一期3个及以上致痛椎的强化术治疗的疗效、安全性及术中注意事项。方法〓回顾性分析采用一期3个及以上致痛椎的强化术治疗的骨质疏松性椎体压缩骨折患者26例,分析患者术前、术中及术后临床及影像学资料。采用视觉模拟评分(VAS)评价患者术后疼痛缓解程度。结果〓一次手术完成3个椎体强化17例,4个椎体强化8例,5个椎体强化1例。术前VAS评分平均为8.48±1.36,术后2天平均为3.08±1.24;术后3个月平均为1.96±1.17,末次随访平均为1.65±1.25,疼痛缓解明显。术后出现骨水泥少许向椎体前方外溢者7个,进入周围静脉丛者1个,但均无异常临床表现。结论〓3个及以上椎体一次性椎体强化术临床安全性高、疗效确切,在充分评估患者可以耐受的基础上可考虑施行。

Abstract: 【Abstract】 Objective To evaluate the therapeutic efficacy,safety and surgical technique and tips of one-stage percutaneous vertebral augmentation of three or more vertebral body for multiple-level osteoporosis vertebral compression fracture. Methods A retrospective study was conducted to review twenty-six osteoporotic vertebral compression fracture patients who received one-stage percutaneous vertebral augmentation of three or more vertebral body. The outcome was considered by preoperatively and postoperatively by the Visual Analogue Scale (VAS) for pain relief. Clinical and image information of pre- and post-operation also be analyzed. Results All surgical procedures were performed successfully. The number of operating vertebrae in one time was 3,4,5 in 17,8,1 patients, respectively. The VAS changed from a mean preoperative score of 8.48±1.36 to a mean score of 3.08±1.24 on postoperative two days,1.96±1.17 three months after operation and 1.65±1.25 at final follow-up. Slight cement leakage was found in seven patients and one case was found the cement was leaked into perivertebral venous plexus, but without any clinical symptoms. Conclusion One-stage percutaneous vertebral augmentation of three or more vertebral body is safe and effective. It can be used in patients who are able to tolerate the surgery followed by professional assessment.
