Abstract: Objective To discuss the operative method and clinical effect of pneumonectomy plus partial resection of left atrium in locally advanced lung cancer. Methods From Dec. 2001 to Dec. 2006, 16 patients with locally advanced lung cancer involving left atrium received partial resection of left atrium plus pneumonectomy were analyzed retrospectively.In these cases, 6 underwent the left lower lobe resection, 4 underwent left pneumonectomy, 3 underwent right and lower lobe resection, 3 underwent right middle lower lobe resection. Results No operative death or intraoperative complication was found. Arrhythmia happened in 2 cases, pneumonia in 1 case. The average survival period was 36 months. The postoperative survival rates of 1,3,5- years were in 75%（12/16）、43.75%（7/16）、25％（4/16）retrospectively. Conclusion Pneumonectomy combined with partial resection of left atrium and postoperative chemotherapy have a good clinical effect, and would improve the patients’survival quality and prolong the survival time.
